Most of the time when I review my clients’ medical records I notice that the doctors will often be extremely short in their description of the patients’ injuries.

Most doctors do this to save time and money, but ultimately this may destroy your case. The history and descriptions you give your doctor about your medical condition must be detailed, truthful and complete.

I actually have an eight-question worksheet that I have most of my clients use to assist them in telling the doctor what the doctor needs to know about their history and description of their injuries.

If you would like to see that worksheet, click here for a print-friendly copy.

If you want to comment about how a doctor has inaccurately recorded your medical conditions, I’d love to hear from you in your comments below on this topic.

A few months ago my grown daughter had a seizure in Walmart and fell backwards hitting her head on pole which held a fire extinguisher. The fire extinguisher fell off the pole and hit her in the back of the head causing a nasty head injury. She was knocked unconcious and had to be taken to Wesley by ambulance. Walmart has denied any medical claims on this accident. The injury occured not as a result of the seizure but as a result of the fire extinguisher hitting her. Does she have a case for a law suit?

I was driving a truck hauling haz/mar (gasoline). The vehicle that caused the accident was underinsured.Because i was put on disability afterwards,I received work comp. and ask to fall back on my company liability coverage. Which is a 1,000,000 in coverages.Hauling haz/mat my truck falls under the (fmcsa) federal motors carriers safety adm. My company has said they lowered the liability coverage under kansas law 40-284.The fmcsa has told me they can not mix kansas law and override federal law.No where does the company i worked for do they show a rejection. Not with the K.C.C or FMCSA. The companies form 91x clearly shows the 1,000,000 with no waivers to BIPD. The fmcsa does talk about the driver and cargo being covered under the fmcsa law 387.9. I believe the driver is covered under the definition of bodily injury and not public liability. The company says that they can reject the limits and lower them to kansas law 40-3107. Can you help? What is true?
